Ingrown Toe Nails Medical Treatment
Medical Care:
Treatment options depend on the stage of ingrown toenails, medically known as onychocryptosis.
Stage 1 can be managed by recommending shoes with a comfortable wide toe box or open-toed shoes. Instruct the patient’s parents to cut the nail straight across and avoid cutting back the lateral margins. The nail edge should extend past the tissue.
Stage 2 can be treated by stretching the soft tissue away from the side of the nail, elevating the offending edge of nail from the soft tissue, and placing a small pledget of cotton under the nail edge to lift it back into the nail grove. Instruct patients with stage 2 ingrown nails on how to perform this treatment. Parents should also be instructed to have the child rest, keep the foot elevated, and use warm soaks.
Stage 3 should be treated by removing the nail margin as described in “Surgical Care.” Chronic ingrown toenails may require matrix ablation.
Surgical Care:
Stage 3 ingrown nails require avulsion of the lateral border of the nail plate with sharp excision of the hypertrophic granulation tissue. If avulsion has been unsuccessful in the past, partial or total ablation of the nail plate chemically, surgically, or via laser may be indicated.
Prepare the digit with Betadine or alcohol if the patient is iodine allergic. Perform a digital block with 2% lidocaine without epinephrine.
Lift the nail off of the nail matrix bluntly all the way back to approximately one eighth of an inch under the proximal nail fold.
Insert a scissors blade and cut the nail back to the proximal nail fold.
Remove the free portion of the nail.
Protuberant granulation tissue can be removed sharply or treated with silver nitrate.
Bleeding, if any, is controlled with pressure.
Antibiotic ointment and clean dressing should be applied.
Consultations:
Consult a podiatrist for routine follow-up care or for patients in whom primary avulsion therapy has been unsuccessful.
Close follow-up care with an orthopedist is required if inflammatory osteophytic changes are observed or if evidence of osteomyelitis is present.
Follow-up with a primary care physician is indicated for any type of immunosuppression, including diabetes mellitus.
Diet: No dietary limitations are required.
Activity: Rest, keep the extremity elevated, soak the affected nail in warm water, and maintain limited weight bearing until healing has taken place.

New Products and Inventions From Old
A fast way to invent new things is to start with existing concepts and find new applications. With the concept of inflatable things, the first thing that comes to my mind is inflatable shelters for emergency situations, such as after earthquakes or hurricanes. Such shelters could be transported easily, and erected quickly with a simple air pump. A basic large tent design, but with inflatable ribs instead of poles might work.
We can always find new products and inventions for babies. Inflatable cribs or playpens come to mind. Deflated, they could be folded up and stored almost anywhere. A simple design for a playpen might be a plastic floor with a simple wall that surrounds it and is attached to it. Think of air mattresses for swimming, set on their sides, connected end to end and wrapped into a circle and you’ll get the idea.
When we used to go “tubing” down rivers in Michigan, we were forever trying new ways to carry a cooler with us while keeping it convenient to get a soda or beer out of it. A solution could be an inflatable bar. It might have a cooler built into it, have can and glass holders, and maybe even a secure place to set snacks. It could be used in a pool, lake or river.
Many people drive into water and die each year. One solution to this problem might be inflatable flotation that is activated when the car begins to sink. They would quickly inflate in an accident involving submersion, and would keep the car afloat. Put one in the trunk, and another inside the car, or have them come out from the wheel wells. To avoid accidental inflation, the triggering device would be activated by water, but be in a place where rain and car washing water couldn’t reach.
Vending Machine Products and Inventions
When I considered the concept of vending machines, the first thing that came to mind was a beer machine. It could only be used in a bar that was restricted to adults, of course. A big benefit is that it would lessen the need for bartenders, since half of all the drinks sold in a bar are just simple beers. It is an innovation that could be implemented tomorrow, using beer in cans in existing pop machines.
A vending machine for books and magazines might do decent business in an airport, bus station or other places where people are forced to sit for hours. They may already exist, but I haven’t seen them yet. Add some padding to the drop chute, and existing snack vending machines could be used.
They sell phone cards everywhere now, but I haven’t yet seen them in vending machines. You could also sell those collapsible umbrellas from a machine. There are dozens of other things that could be profitably sold from vending machines. Use simple techniques like this extension of existing concepts, and there are also thousands of other new products and inventions waiting to be made.

When planning a trip to California, you might as well spend a good portion of it in San Diego. This city, while famous for its trolley and being the home of the world’s most crooked road, offers more than just curiosities. Take the time to explore and it’s guaranteed to be a trip well worth the time. Here are just some places to get you started.
San Diego Zoo
The world-famous San Diego Zoo began as a collection of plants and animals from the Panama-American Exposition back in the early 1900s. Today, it is a world-class research facility and home to several endangered species, all the while still a fun place for families to go to on vacation.
With such wide acreage, going through the entire zoo will take more than just a day trip. Fortunately, there are several benches all over the zoo to give rest tired feet a rest. Mei Sheng, the panda, is a tourist favorite and the queue can take as long as an hour during peak season. Zoo mascots are around for photo ops with children (and adults, if they’d like too!).
You can get there by car, bus or bike and the parking is free. You can enter via Zoo Place just off Park Boulevard, which is located in Balboa Park, close to downtown San Diego.
Balboa Park
Balboa Park lies northeast of downtown San Diego. The park boasts of hiking and biking trails as well as play and picnic grounds. The Sixth Avenue and Laurel Street entrance has a bridge you can cross, which will lead you to what is left of the Panama-American Exposition held in 1915 to 1916. The buildings that house shops and museums in Old Town take to the architecture of the period of California’s beginnings. And to keep with the tradition of laying back and taking in the scenery, it is best to set aside a day to just explore and enjoy the town. Old Town can be reached using the San Diego Trolley that are marked red or take 10 short minutes from the Gaslamp Quarter.
Gaslamp Quarter
What used to be an area you didn’t want to be in after dark is now a safe and fun place to spend both the day AND night. The Gaslamp Quarter is the result of San Diego City taking charge and saving a community from being run down. Now even the San Diego Repertory Theater calls this part of the city home.
Enjoy the place by just walking around and taking in the local color. You can even take a horse-drawn carriage or ride the San Diego Trolley. If you are in the area some time September, then you might want to check out the “Street Scene” which is a music, food and drinking festival that lasts for three days.
La Jolla
If nature tripping is your kind of vacation, then by all means visit La Jolla (pronounced La Hoya). The rarest pine trees in the country call this place, just 30 minutes from downtown San Diego, home sweet home.
Hikers and trekkers will enjoy the path that takes them up cliffs and down the beaches. The village offers fine art museums and galleries as well as fine dining and shopping. A hike up Mt. Soledad will give you a panoramic view of San Diego City down below.
