Ingrown Toe Nails Medical Treatment

Medical Care:

Treatment options depend on the stage of ingrown toenails, medically known as onychocryptosis.

Stage 1 can be managed by recommending shoes with a comfortable wide toe box or open-toed shoes. Instruct the patient’s parents to cut the nail straight across and avoid cutting back the lateral margins. The nail edge should extend past the tissue.

Stage 2 can be treated by stretching the soft tissue away from the side of the nail, elevating the offending edge of nail from the soft tissue, and placing a small pledget of cotton under the nail edge to lift it back into the nail grove. Instruct patients with stage 2 ingrown nails on how to perform this treatment. Parents should also be instructed to have the child rest, keep the foot elevated, and use warm soaks.

Stage 3 should be treated by removing the nail margin as described in “Surgical Care.” Chronic ingrown toenails may require matrix ablation.

Surgical Care:

Stage 3 ingrown nails require avulsion of the lateral border of the nail plate with sharp excision of the hypertrophic granulation tissue. If avulsion has been unsuccessful in the past, partial or total ablation of the nail plate chemically, surgically, or via laser may be indicated.
Prepare the digit with Betadine or alcohol if the patient is iodine allergic. Perform a digital block with 2% lidocaine without epinephrine.
Lift the nail off of the nail matrix bluntly all the way back to approximately one eighth of an inch under the proximal nail fold.
Insert a scissors blade and cut the nail back to the proximal nail fold.
Remove the free portion of the nail.

Protuberant granulation tissue can be removed sharply or treated with silver nitrate.
Bleeding, if any, is controlled with pressure.
Antibiotic ointment and clean dressing should be applied.

Consultations:

Consult a podiatrist for routine follow-up care or for patients in whom primary avulsion therapy has been unsuccessful.
Close follow-up care with an orthopedist is required if inflammatory osteophytic changes are observed or if evidence of osteomyelitis is present.

Follow-up with a primary care physician is indicated for any type of immunosuppression, including diabetes mellitus.

Diet: No dietary limitations are required.

Activity: Rest, keep the extremity elevated, soak the affected nail in warm water, and maintain limited weight bearing until healing has taken place.

Plan To Succeed!

To get what we want in life we need to have a plan. Goals are an
important part of making plans and being successful. If we want
to lose weight, we need to set goals and have a plan. If we want
to be fit, we need to set goals and have a plan. If we want to
have less stress, we need to set goals and have a plan. You’ve
heard that saying? “She that fails to plan plans to fail.” So
how can you plan to succeed? You need to start by setting goals
following the S.M.A.R.T. formula.

Most people say things like, “I want to be thinner” or “I want
to get fit” or “I want to be healthier, happier, less stressed
etc.” These are not goals you can plan to achieve. They are
wishes or desires. S.M.A.R.T goals are:

S = Specific M = Measurable A = Action Based R =
Realistic T = Time Bound

Be specific about what thinner, fitter, healthier or less
stressed means to you. Make your goal something you can measure.
Being thinner means losing how much weight or being fitter means
running how many kilometres? What does less stress involve for
you?

Make sure you will get into action about your goal by basing it
on a true desire. If you are not excited about your goal, find
ways to make it more fun. If you’re trying to lose weight then
save all the money you usually spend on chocolate and put it
towards the reward of a holiday or new clothes.

Be realistic about your goal. Losing two or three kilos a week
may be difficult and unhealthy, so plan to aim to lose a half a
kilo or one kilo a week instead. If the goal is realistic you
will stick to it.

Lastly make your goal time bound by giving it an end date. This
gives your goal a sense of urgency and makes you get into action
about it. Keep this date realistic.

This week look at your goals and make them S.M.A.R.T. “I will
lose 20 kilos by January 28th 2006″ “I will be able to run 5
kilometres in 15 minutes by February 16th 2006″ Above all don’t
fail to plan. Plan to succeed!

Sometimes, the army we sent to the site of an injury or infected area, mistake our body parts for invaders. Instead of this army destroying bad guys, it focuses on destroying us.

Old Ideas

For many years, researchers thought of heart and vascular disease as a “plumbing problem.” The patient just consumed too many steaks and the fat clogged their arteries.

The solution therefore was to take a bunch of pills to “unclog the pipes.” Or, if the doctor was old fashioned, they might recommend lowering the consumption of fatty foods. After all, a modern doctor uses pills to make people well.

But how do they explain the heart patient with a low level of fats in their blood? It’s like trying to explain away the AIDS patient with NO HIV virus. “Er, ah, ah er well take the pills anyway. And don’t forget to pay the receptionist on the way out.”

4d and 3d scans - baby ultrasound company

The method know as three dimensional ultrasound scanning is used in early pregnancy, it provides 3d pictures of the unborn baby. Most times these ultrasound samples are collected and combined and animated to produce a 4d scan.

Three dimensional scanning works in the same manner to the normal scanning methods except that the ultrasound scanning pulses can be directed from multiple directions. The ultrasound pulses can be reflected back and captured and provide information to construct a 3-dimensional image in much the same way as 3d movies. 3 dimesional ultasound scanning was devised by olaf ramm and stephen smith.

It’s important to understand that sonologists worldwide have always conjured 3d pictures of the body in their minds while doing 2d scans. However, until recently it was not possible to do this kind of reconstruction on on data using ultasound scanning. With the introduction of baby scans for the first time allowed us a peek into the brain of a sonologist and so allowing us see the images on the ultrasound machine.

3d/4d imaging should utilize ultrasound energy following the same limits as conventional 2d ultrasound to create the 3d images. While there is no information of harm due to 3d ultrasound, its use in non-medical situations needs to be undertaken with an understanding of the risks that may exist.

Bird Flu: Personal Preparedness Must Include These 4 Critical Areas

The recent hurricanes Katrina and Rita have been powerful
reminders of how destructive the forces of nature can be, and
how preparation for them can mitigate their effects. Avian
influenza, commonly referred to as “bird flu,” is a powerful
force of nature that we must prepare for–or suffer the
potentially devastating health and financial consequences. Bird
flu is a viral contagious disease, just like the regular
seasonal flu, but it might turn out to be 70 times more deadly.
And, because of the nature of the virus, it might be most deadly
for healthy children and adults, and pregnant women–just like
the so-called Spanish flu of 1918-19 was.

The report of the U.S. National Intelligence Council’s 2020
Project, Mapping the Global Future, identified a global pandemic
(an epidemic that is worldwide) as the single most important
threat to the global economy. According to Shigeru Omi, regional
director of the World Health Organization, “The world is now in
the gravest possible danger of a pandemic.” And according to Dr.
Robert Webster, a world-renowned influenza researcher at St.
Jude’s Children’s Research Hospital in Memphis, Tennessee, “We
could be heading for a global catastrophe.” Infectious-disease
experts have repeatedly warned that it’s not a question of
whether a bird-flu pandemic is coming; it’s only a question of
when.

Judging from the federal government’s incredibly inadequate
response at all levels to hurricane Katrina–which is emblematic
of its ineptness in dealing with large national emergencies, its
slow and superficial response to bird flu to date, and its lack
of leadership on this issue–it is clear that you cannot count
on the government to protect you. You must take the initiative
to prepare yourself and your family for the coming bird-flu
pandemic.

There are four essential areas that you must address to prepare
for the bird-flu pandemic: 1) “social distancing”; 2)
commodities–including food, 3) personal protective equipment
(PPE), and 4) financial preparation. Social distancing refers to
your living and work situations when the pandemic strikes.
Without going to extremes, you want you and your family to be as
far away from other people as possible. Bird flu is just like
the regular seasonal flu in that you become infected from other
people, not birds. (Although it might be possible to acquire the
viral infection from birds, it is much more likely that, if you
do become infected, you will have acquired the virus from
another person, not a bird.)

The bird-flu virus is extremely contagious; it is transmitted
though casual contact with a contagious person (who might not
have any symptoms during the first 24 hours of infection),
through touching contaminated objects, and through the air.
Because of this, you want to stay away from people as much as
possible, and that means spending more time at home. Your
children will not be at school, they will be home. If your home
is on the 73rd floor in an apartment building in New York City,
how are you going to avoid other people? You might want to think
of an alternative living situation for a few months.

The same principle applies to your work setting. If you can
telecommute, that is the best scenario. If you don’t telecommute
now, but because of the type of work you do it might be a
possibility, discuss it with your employer. If you will have to
continue to work closely with others at your job site, what can
be done there to help protect you and others from infection? How
can policies and procedures be amended to minimize contact with
coworkers or customers? Are there hand-washing stations
available? What are your employer’s plans for dealing with the
coming pandemic? Discuss these and related issues with your
employer and coworkers.

The second area that must be addressed is
“commodities–including food.” There will be sporadic
difficulties manufacturing or producing goods–because workers
around the globe will be sick or otherwise absent from work.
There also will be supply chain disruptions–both because
workers will be sick or otherwise absent from work, and because
of regional, national and/or international restrictions on
travel. These problems will cause a decrease or the
unavailability of most or all of the products we easily have
access to now.

Commodities such as soap, toothpaste, toilet paper, and
virtually everything you can buy at stores such as Wal-Mart will
be difficult or impossible to obtain–for periods of weeks or
months at a time. This includes the most important
commodity–food. The federal government is always telling us to
stock up on emergency supplies for three days. This will not be
sufficient preparation for the coming deadly bird-flu pandemic.
There will likely be limited food available in stores. In
addition, stores are places you want to avoid anyway, because
potentially contagious people might be there. Stock up now so
that you have sufficient commodities, including food, for a
period of months.

The third area to address is so-called personal protective
equipment (PPE), which you will have to use, depending upon
circumstances. PPE includes special face masks, called N95
respirators, which help prevent infection through inhalation of
the virus. Remember that avian influenza (”bird flu”) is a very
contagious disease that can be transmitted through the air. The
only way to counter this source of infection is through the use
of special N95 respirators. These are disposable face masks that
can be worn for up to eight hours.

N95 masks were the type of masks worn by hospital workers during
the 2003 severe acute respiratory syndrome (SARS) pandemic that
struck a number of cities around the world, including Toronto,
Canada. Surgical masks or other common face masks, sometimes
used when sanding or painting and so forth, are not effective.
Other elements of PPE include disposable latex or vinyl gloves,
eye goggles or face shields, gowns impervious to liquids, and
sometimes disposable booties or disposable head caps. Keep in
mind that during the pandemic, most people who become infected
will have to be cared for at home, not in crowded and
overwhelmed hospitals. This means that caregivers taking care of
loved ones at home need to be protected from the virus, just
like hospital workers working in hospitals. The only way to be
protected is to wear PPE. (Just washing your hands–the federal
government’s primary recommendation for caregivers at home–will
not be enough.) Once the pandemic starts, demand for PPE will be
huge and supplies will be in very short supply–or nonexistent.
Buy now or suffer the consequences later.

The last area that must be addressed before the bird-flu
pandemic strikes is personal finances. This is an area that
governments at all levels have been mute on. However, preparing
your finances to sustain yourself and your family during (and
after) the pandemic could prove to be the most important area of
preparation. Although the bird-flu virus is deadly and many of
us will fall ill, most of us will not die from it–only one to
two percent of the population will likely die. The vast majority
will live–but under what circumstances?

Think of hurricane Katrina–where most people survived–but
where hundreds of thousands are now homeless and underemployed
or unemployed. Because of the potentially severe local,
national, and international economic consequences of the
bird-flu pandemic, many of us will suffer financially.
Businesses around the world will not be able to make or
distribute products or provide services. There will be layoffs
and some companies will go out of business altogether. At a
minimum, people will be out of work for periods of weeks or
months. Your child or children, if you have any, will be at
home–not in school or day care. Will that force one parent to
stay home from work to care for them? How will you pay your rent
or mortgage and your bills under these circumstances?

At Benjamin Franklin said, “An ounce of prevention is worth a
pound of cure.” When it comes to bird-flu preparation could make
the difference between life and death, how much you and your
family eat, and whether or not you can pay your bills, including
your rent or mortgage. The government will not resolve these
issues for you. Just like Smoky the Bear’s admonition, “Only you
can prevent forest fires.” Only you can take stock of this
situation and do something about it. Think about it–and then do
something about it.

Good Bacteria: Probiotics and Prebiotics

There are two absorption systems in your body. Food that is easily broken down is absorbed into your upper intestinal tract. If you cannot absorb a particular food, it goes to the lower intestinal tract (colon), where bacteria (in your colon) ferment it to smaller products that can be absorbed.

Your colon is loaded with good and bad bacteria. Examples of bad bacteria are clostridia that are kept in check by good bacteria. If you take an antibiotic that knocks off the good bacteria, the clostridia can overgrow and cause horrible bloody diarrhea and you can die if you are not given special antibiotics, specific to kill clostridia, such as metronidazole and vancomycin.

The good bacteria break down soluble fiber to form chemicals such as short chain fatty acids that are absorbed into your bloodstream and travel to your liver where they block the liver from making cholesterol and help to prevent heart attacks. These short chain fatty acids also reduce inflammation, so they help to control the horrible bloody diarrhea and ulcers caused by Crohn’s disease. They also reduce swelling and pain of arthritis, diabetes and psoriasis, and some studies show they may even improve your immunity to help you to kill germs.

One example of friendly bacteria is lactobacilli, found in live-culture yogurt. However, lactobacilli do not thrive in your intestines and disappear if you don’t eat yogurt every day. Most commercial yogurts do not contain live cultures; check the labels if you seek this benefit.

Foods that cannot be absorbed in your upper intestinal tract pass to your colon and form the food for the huge colony of friendly bacteria there. These include whole grains, beans, seeds, vegetables and nuts. The part of these foods that encourages the growth of good bacteria is most likely the soluble fiber.

So probiotics are living microorganisms, the good bacteria, that live in the colon and reduce inflammation and help prevent and treat Crohn’s disease, psoriasis, arthritis, and perhaps even certain types of cancers.

Prebiotics are the nondigestible food ingredients that cannot be absorbed in the upper intestinal tract and travel to the colon to encourage the growth of these good bacteria. A healthful, high-fiber diet will include a wide variety of these foods.
